....who foots the bill? Does the school have an agreement to pay for it, or does the player and his family have to go out of pocket for it?
 
If the surgery is because of an issue caused by a sport, I'm sure the school's insurance covers it. If it is not related, probably not.
 
Insurance will generally cover such things. We do get some insurance as students, but I'm not sure what kind of surgery coverage it provides.

Adding a bit, the school insurance is voluntary and must be payed for (just to clarify that it wasn't part of basic student stuff). Not sure on coverage though, all I can say is it is the only insurance the Health Center Pharmacy takes.

i had an ACL while on the student insurance. it is a major medical plan, plus you get the walkin treatment at the center. pretty strict in my day about going there first before seeing another doctor

they paid out over $20k for my ACL back in '89 -- i think the AA carries separate insurance, but i am not sure
 
The player's primary insurance pays (usually through a parent); the school picks up the rest. The player should have no out of pocket expense.
The players have 18 months after their eligibility is exhausted to have any reported injuries resolved.
